Transitional Care Facility
Worcester TCF ward: 18 beds
Ceres TCF ward: 11 beds
Our TCF services are provided 24 hours a day.
These services are nurse-driven and supported by a team of professionals: doctors; social workers and allied health care workers (physio-; occupational and speech therapists).
Home Based Care
Visits by members of our team to you in your home. This service is provided Monday to Friday throughout the day.
Community Health Worker with counselling
Access the services of these lay counsellors through the Department of Health facilities
